2009 Distinguished Alumni Service Award Nomination Form

The Distinguished Alumni Service Award is the highest honor the Alumni Association can bestow upon an alumnus/a on behalf of the University.

Recommendations will be accepted by the University community until Friday, February 20, 2009. Download a printable form, or submit the online version below.

DASA nominees will be evaluated on the following criteria:

  • Level of commitment – a high level of dedication to the University, the depth of involvement, and the amount of time committed. Service of all types is considered, whether working with students, providing leadership to the University and/or the Alumni Association, or serving the University in unique capacity
  • Actions taken in support of the University and/or the Alumni Association – service that helps preserve, maintain and promote the traditions of the University
  • Impact – amount of impact the nominee’s service has had on the University and/or the Alumni Association

The award is not intended to solely celebrate famous alumni, nor to recognize a distinguished professional career. The DASA cannot be given posthumously or be presented to an individual who is an officer, faculty member or employee of the University or is a member of the Board of Regents. The award is presented each fall, and the recipient must be present to win.

Nominee's information

Describe the type and extent of volunteer involvement this person has had with the University of Michigan and/or the Alumni Association (i.e. governing boards, school/college activities, local alumni club involvement, fundraising, etc.). Provide specific examples of the candidate's service to the University.

List names and contact information of at least two (2) other individuals (deans, faculty, staff, board members, etc.) who are familiar with this person's volunteer service.

List names and contact information of at least two (2) other individuals (classmates, friends, fraternity/sorority members, etc.) who know this person.

Letters of nomination should be addressed to DASA committee members (but sent to the attention of Shannon Wilson). All supporting documentation must be postmarked no later than Friday, February 20, 2009.
